Job description
This role is placing Home Health LPNs across the greater Philadelphia area.
You will complete in-home visits, follow care plans, and keep the case manager informed after each stop.
What the week looks like
- Drive between patient homes (mileage discussed)
- Perform skilled nursing visits per plan of care
- Teach patients/families medication and wound care basics
- Submit visit notes same day
Requirements
- PA LPN license
- Valid driver’s license and reliable transportation
- Home health experience preferred
- Self-directed and organized
Pay
- $28–$34/hr
- Daytime visit schedule
