    Job description

    This role is placing Home Health LPNs across the greater Philadelphia area.

    You will complete in-home visits, follow care plans, and keep the case manager informed after each stop.

    What the week looks like

    • Drive between patient homes (mileage discussed)
    • Perform skilled nursing visits per plan of care
    • Teach patients/families medication and wound care basics
    • Submit visit notes same day

    Requirements

    • PA LPN license
    • Valid driver’s license and reliable transportation
    • Home health experience preferred
    • Self-directed and organized

    Pay

    • $28–$34/hr
    • Daytime visit schedule
